Summary
BSEE investigates a tubing-lift injury during well abandonment at West Cameron 180G. Contact with a tong arm caused the joint to strike a worker’s chest and then land on his ankle, fracturing it. Findings examine blind lifts, unmarked danger zones, conflicting job safety analyses and failures to stop unsafe work.
