Summary
Investigation of a contractor’s finger and thumb injuries while disconnecting a vacuum hose blocked with mud and cuttings. The hose fell against a pipe, trapping his hand. Findings identify missing job safety analysis, inadequate planning and coordination, and departure from procedures; recommendations address refresher training, chain-hoist use where possible and reliable assistance.
