Summary
An investigation records a heat-tracing fire at the Bullwinkle platform. Damage or wear to the element was identified as the probable cause. A high-impedance arcing fault did not trip the breaker, and flames repeatedly returned after extinguishing. Electrical isolation stopped reignition; all personnel were accounted for during muster.
