Summary

A contractor suffered a compound elbow fracture when malfunctioning cycling gear caused an automated derrick’s bails and elevator to rotate and strike him during drilling. The investigation identified gear malfunction as the probable cause and inadequate no-go-zone width as a contributing factor. BSEE made no recommendations to the Agency.
