Summary
BSEE investigates a technician’s left-hand injury during BOP soak testing aboard the Discoverer Inspiration drillship. A hose pressurised to 5000 psi ruptured when moved, injecting BOP fluid. Findings address possible internal wire damage and corrosion, deficient hose maintenance, inadequate procedures and hazard recognition, and failures in communication and access restriction.
