Summary
The investigation examines tensioner-ring disengagement during slip-joint extension on Noble Globetrotter. Incompletely engaged locking dogs and a secondary lock left the riser unsupported, causing extensive equipment damage. Findings identify absent engagement verification, unfamiliarity with checking procedures and vague work instructions. A top-drive guide plate also fell forty feet to the rig floor.
