Summary
Investigation of a finger latch and retention cable falling approximately 101 feet during drill-pipe racking on Diamond Offshore Black Lion. No injuries were reported. Findings address inadequate visibility, departure from the site-specific plan, ineffective retention and faulty latch-status data. Subsequent actions included additional retention and trials of alternative latch arrangements.
