Summary
During plug and abandonment, a telescoping swivel sub parted and trapped pressure expelled approximately 500 feet of tubing. Mismatched component designs and buckled internal parts were identified. The well was controlled without injuries. Recommendations address worst-case tool testing and, whenever feasible, killing the well and bleeding trapped pressure before unseating the completion string.
