Two accidental marine riser disconnects involved selection of the wrong control button and installation of a disconnect-switch lock-out device. The latter incident lost riser hydrostatic pressure and surface BOP control. The alert recommends assessing changes and communicating current activities, and stresses reliable backup BOP actuation.
