Safety Alert 247 - Fatality on Storm-Damaged Platform

Summary

A contractor boarded an unmanned, storm-damaged platform for unknown reasons, fell more than 20 feet, struck his head on a lower deck and entered the water; he was later pronounced dead. MMS identified access-control failures and recommended reviewing boarding policies, communicating restrictions, preparing hazard mitigation plans and barricading access.
