Summary
Two employees fell approximately 80 feet into the ocean when a cementitious moisture-barrier layer shattered during platform galley floor repairs; one later died. With its investigation ongoing, BSEE recommends that operators and contractors consider checking similar designs for danger and barricading work areas to restrict access.
