Read at BSEEOpens the publisher’s page on bsee.gov

Summary

Xylene incompatibility was considered a contributing cause, while weather exposure may also have contributed. Rainwater compromised secondary containment. Recommendations address compatible containers, correct labelling, chemical training and inspection for continued service.
