Summary
A pumper suffered head injuries while restarting offshore well production. Pressure against a closed header valve ruptured a flowline weakened by carbon dioxide corrosion; flying debris or impact against an object was reported. The operator’s measures included X-ray, density-meter and visual inspections, corrosion inhibitor injection and replacement with corrosion-resistant alloy steel.
