Summary
Investigation of a short circuit, arc flash and fire during night-time circuit-breaker replacement on Statfjord B. A conductive foreign body was considered the very probable initiating cause. The report examines switchgear protection, maintenance, training, work procedures and emergency coordination. Two people experienced smoke inhalation during subsequent firefighting, rather than direct arc-flash injury.
