Summary
This alert describes two stabbing-basket incidents. In the fatal case, a worker folding the platform lost balance and became trapped; a hoisting chain jammed the air-winch lever on. In the other case, a safety-belt lanyard caught a control lever, raising the assembly and trapping a worker’s leg.
