Summary
During drilling-line reattachment after slipping and cutting, a motorman’s foot slipped from the braking-system equaliser bar into the space between a brake shaft and cam. A floorman inadvertently bumped the crown block safety device, activating the brakes and trapping the motorman’s foot between the brake bar and cam.
