Summary
This alert describes three dropped-object incidents during rig work. A tubing joint caught beneath the top drive fell and knocked off an employee’s hardhat. Casing struck the V-door and caused a shackle failure. During cement-hose disconnection, a sledgehammer fell 35 feet to the rig floor.
