Summary
An assistant cementer sampled slurry using a can attached to a rope in a batch mixer with two agitators. The sampling arrangement wrapped around an agitator shaft, pulling the rope across his right hand and through his glove. Injuries resulted in amputation of two fingers and part of the hand.
