Summary
Two marine riser inserts fell about 15 to 20 feet onto the drill floor while a top drive lifted a pup joint from a raised catwalk. Nobody was injured. The alert describes recent MPI inspection involving insert and circlip removal, and subsequent discovery of four other inserts without retaining circlips.
