Summary
A near miss occurred while laying down 4-1/2 inch tubing using single joint elevators and an air tugger. The elevators opened during lowering, dropping the joint down the V-door onto the catwalk. No personnel were nearby. Following a coffee-break crew change, the returning worker had not checked the latch or safety pin.
