Summary
During foam drilling, compressed air was injected at approximately 800 psi. During a connection, air was diverted without closing the standpipe supply valve. Opening the lower Kelly valve released trapped air through the mouse hole joint, startling a crewmember who slipped and injured his shoulder and arm.
