Summary
A derrickman disconnected his positioning rope to enter a warming hut during a pause in tripping, then failed to reconnect it before handling drill pipe. He fell from the derrick board but was arrested by a self-retracting lifeline. The crew assisted his return using an air hoist line; the harness and lifeline were withdrawn from service.
