Summary
An offshore drilling worker disconnected his safety belt’s hoist line while standing on casing elevators 49 feet above the rig floor. A cementing-head cap blew off and knocked him down, causing fatal injuries. The operator advised personnel to use tied-off safety belts in elevated areas without handrails.
