Crane Rig Up Results in Fall from Height

Summary

During pedestal-crane assembly on an offshore production platform, a hydraulic cylinder slipped from its rigging while being lowered by another crane. It struck a handrail, which broke away and fell to the deck with a derrickman attached by his fall-protection lanyard. He sustained skull fractures and a broken right arm.
