Summary
During rig-up, crane adjustment of a substructure half moved the box tops apart while centre steel was being installed. A previously positioned BOP scaffold came off its track and fell approximately 3.7 metres. Two employees on the scaffold were tied off overhead; their fall-protection equipment worked as intended and neither was injured.
