Summary
A crewman tightened a leaking stand-pipe hammer union during pressure testing. While descending the derrick, he lost his grip on a mud-covered sledgehammer at about 23 feet above the rig floor. The hammer deflected off a brace and struck another worker’s hard hat after the crew re-entered the cleared area.
