Summary
A floorman racking drill pipe on the monkey-board suffered fatal abdominal injuries after slack winch wire allowed the pipe into the descending top drive’s path. The pipe deflected, carrying the wire across the board. The alert leaves uncertain whether the pipe and wire, or only the wire, struck him.
