Summary
This alert describes two separate lost-time incidents involving falling rubber guides from top-drive back-up wrenches. In one, a displaced safety clamp caught the guides and sheared their holding bolts. In the other, lowered blocks hit the stand before the guide bolts sheared; the falling guides injured a worker’s face and cheek.
