Summary
A floorhand suffered fatal injuries when struck by an iron roughneck during drill-pipe tripping. The worker was fitting a collar clamp around a drill collar. Witnesses reported that the remotely operated machine extended into the worker’s path; warning and stopping attempts failed. The investigation remained ongoing.
