Summary
An explosion occurred during the second attempt to lower a measurement while drilling tool into a drill collar. A discharged vent plug shattered the driller’s cabin glass, sending glass and chemicals inside. The driller was struck in the face and neck; other rig-floor employees sustained minor injuries from concussion and gas release.
