Summary
Two vehicle incidents occurred on the same day during rig removal and departure. An extended truck pole struck and broke an overhead ground wire after its height went unchecked. Following a safety stand-down, another truck’s load leaned on a steep shoulder, struck a power pole and brought it down across the lease road.
