Summary
A flagman spotted a loose sledgehammer on a cargo carrying unit being received from a vessel at an offshore platform. Borrowed to clear an obstruction from a mud-loading hose in port, the tool had inadvertently been left on the unit. The platform landing area was secured against entry during cargo operations.
