Summary
An employee unlocking a blowout preventer ram inserted a finger into the OEM sight slot to check that the locking screw was fully withdrawn. The crew chief operated the ram control valve while the finger remained in the slot, resulting in amputation of the left index finger. The hydraulic system was positioned 50 feet away.
