Summary
Two crew members tightening bolts during nipple-up used a three-foot rope to hold a hammer wrench after anticipating a hammer strike. As they moved around the stack, the rope holder entered the line of fire. A glancing hammer blow struck his mouth, causing an OSHA-recordable injury.
