Summary
During tripping operations with 4-inch drill pipe, a racker operator knocked on the cab window to attract the roughnecks’ attention and confirm stand preparation and clearance. The pane became dislodged and fell approximately 13.5 ft to the drill floor, resting on the mouse hole. No one was struck.
