Summary
During casing running, an incorrectly inserted elevator pin preceded full latch closure, allowing the elevators to open while hoisting a joint of 9-5/8″ casing. The joint bounced in the handling trough and fell onto gull wings beside the catwalk. No injuries occurred, but serious injury or equipment damage potential was reported as very high.
