Summary
A subsea engineer on a semi-submersible rig suffered fatal injuries while inspecting a blowout preventer’s ring gasket seating area. The approximately 110-ton BOP was suspended by two 70-ton bridge cranes. Failure of the forward crane’s mechanical brakes allowed it to swing aft, pinning the engineer against the test stump.
