Summary
Testing of a vessel’s helideck foam sample revealed excessive water content, traced to a leaking non-return valve. The diluted concentrate could not have produced the required foam blanket during a helicopter fire. Company actions included leakage testing where possible, frequent planned inspection when convenient, and scheduling an isolation valve.
