Summary
A fatal fall from a high-voltage transmission tower involved a SALA Zorba twin elastic lanyard. Preliminary findings indicated lanyard failure, with early indications of failed stitching at the harness end. Pending investigation, the company withdrew the model and reiterated inspection, documentation and equipment-removal requirements.
