Summary
A crane hook block fell approximately 15 metres during cargo transfer between a vessel and an offshore rig, narrowly missing deck crew. Retrieval of the wrong wire brought the block into contact with the boom. Apparently ineffective limit switches were noted; actions addressed clear decks, communication, testing and readiness to stop unsafe work.
