Summary
A crated circuit breaker fell into a vessel’s engine room during lowering through a hatch. Vessel roll brought the load against ductwork before it tipped and a choker sling slipped off. Nobody was injured. Findings addressed unsecured contents, slinging, vessel motion, risk assessment, supervision and lifting training.
