Summary
This safety flash describes a scaffolder falling after a supporting vertical tube slipped through its coupler. Nearby workers could not tighten the connection in time. An inertia reel harness arrested the fall, while the scaffolder retained the tube and remained suspended over water for approximately seven minutes.
