Summary
A forklift used to tension an electrical cable remained connected to a worktable after testing. A subsequent operator drove away, dragging the table approximately one metre before stopping. Nobody was injured. Findings highlighted missing procedures and job safety analysis, poor communication and omitted vehicle checks.
