Summary
Two anchor-line wire inserts failed on a semi-submersible accommodation unit during extreme weather. Recovery revealed wires slipping from rig-end sockets. Investigation identified improper socket moulding, absent formal training and unaudited suppliers. Recommendations addressed qualification, supplier quality audits, incident sharing and research into testing moulded sockets; buoy support was not established as causative.
