Summary
A crew transfer vessel experienced seawater ingress while alongside, with a cracked engine cooling return pipe initially identified. Further fault finding traced continued flow to the water-lubricated stern tube. An unhighlighted seacock lacked easy access. Crews of similar vessels were required to inspect plastic pipework regularly for deterioration or stress.
