Summary
A safety flash describes lifting chains falling from an offshore wind turbine onto a crew transfer vessel, without injury. Manual repositioning intended to speed up lifting led to the crane hook handle striking the transition-piece gate handrail, opening and tilting the hook and releasing the chains.
