Summary
This safety flash describes a lifeboat recovery incident during routine planned maintenance. After personnel re-boarded the vessel, a noise prompted an immediate stop with the boat approximately one metre above deck. Inspection found a disintegrated davit sheave perimeter, which had allowed the falls wire to leave the sheave and become trapped.
