Summary
An A-frame moved unexpectedly during umbilical re-attachment, lifting the tether management system approximately one metre off the deck while a technician was on top. No injuries or equipment damage occurred. Investigation identified a failed proportional valve driver. Subsequent changes addressed hydraulic isolation, work procedures, risk assessments and control-system protection.
