Summary
Cable socks supporting a subsea dewatering hose slipped along a winch wire, overloading and breaking socks above. Investigation identified unsuitable grip, swell-related loading, an unrepresentative deck test and inadequate assessment of a later rigging change. Replacement rigging using support-wire clamps enabled successful completion of dewatering.
