Summary
An experienced seafarer sustained a severe open thumb fracture while closing a daughter craft’s cabin door during a casualty-handling drill. The investigation identified lost situational awareness and poor judgement. Actions include checking possible preventative door-design changes and using a Time Out for Safety to highlight the trapping hazard.
